ASSESSMENT

The first step in any effort to assist someone who is struggling with addiction, substance abuse and / or behavioral issues is to obtain a thorough history of not only the individual of concern but also of their interaction with other family members, friends or even coworkers. This gathering of information is highly personal and always treated with the strictest confidence.
From that information we are able to then provide a plan for moving forward which will consist of appropriate recommended levels of care, several options for treatment (based on program expertise, age, gender and financial commitment) as well as the appropriate type of intervention best suited for the individual of concern.
INTERVENTION
We take the uncertainty and questions of “What to Do” or “How Do We Do It” out of the equation, leaving you the ability to focus all of your attention on truly providing them with an opportunity for help.
An Intervention, in its simplest form, is the process of disrupting disruptive behavior. Once we have completed the assessment process and the family has selected a course of treatment we will then work with you to identify and invite individuals who would be appropriate to participate in the intervention. During this phase we will also work with you to establish all time lines and meeting venues with regard to the prep-work and intervention as well as set all the logistics (to include travel if needed) for the admissions into treatment.
The intervention process is about moving those closest, out of the chaos and frustration and into a healthy relationship while supporting the individual of concern with a specific opportunity to seek out recovery, healing, freedom and growth. This is all done in a non-confrontational manner and always with respect. Once the admissions process has been completed we will monitor the individuals progress and assist the family members should there be any questions or concerns.
CARE MANAGEMENT
Relapse rates for addiction resemble those of other chronic diseases such as diabetes, hypertension and asthma. Like other chronic diseases, addiction can be managed successfully. National Institute on Drug Abuse (NIDA)
Nothing is more important (prior to leaving treatment) than to have a plan for continuing growth and recovery… Experience has shown us that the majority of people who relapse do so because they did not have a solid aftercare plan in place, or they did not have proper management of their plan. Our Care Management works with the existing clinical team as well as the individual in treatment to identify their specific needs (support groups, sober living, individual therapists, nutritionists, personal trainers, chiropractors, additional medical personnel or employment services) and create a program of accountability for a period of 4-6 months.
After an individual has completed their primary treatment program, our Care Managers will meet with them to establish their individual plan and conduct one-on-one weekly reviews to track their progress, identify areas of concern and make adjustments as needed. As each individual grows in their recovery, their plan must also be able to grow and adapt to new challenges or needs.
